Not medical advice, and not for emergencies

• The site gives general information about physical therapy at Flexlab. It is not medical advice. Reading it, or sending us a form, does not make you our patient: your care starts when one of our physical therapists evaluates you.
• In an emergency, call 911 or go to the nearest emergency room. Do not use the site, email or text for anything urgent.

Who the site is for

• The site is for people in the United States.
• If you are under 18, a parent or guardian needs to book for you and fill in your forms.

Booking and forms

• Give true and complete details. Fill in forms only for yourself, or for someone you have the legal right to act for, such as your child.
• A request is not a booked visit. A time is yours once you book it online after we confirm your intake form, or once we agree it with you directly.
• To change or cancel a visit, call or text (978) 981-1840 at least 24 hours before. The financial and cancellation policies you agree to on the intake form also apply.
• The links and codes we email you, such as for your intake form, to book or to sign in, are for you alone. Please do not share them.

For physicians and other providers

Send a referral only for a patient in your care, with details that are correct as far as you know.

Using the site fairly

• Do not test the site's security, try to reach parts of the site or our systems that are not meant for you, disrupt the site, or send forms or files automatically.
• Upload only the forms we ask for, as a PDF or a photo. Do not upload anything harmful, or anything about someone else that you have no right to share.
• Do not use our forms to advertise or sell anything.
• We may block anyone who breaks these terms from using the site.

Our name and content

The Flexlab name and logo, and the words and pictures on this site, belong to Flexlab Physical Therapy or to those who license them to us. You may view and print pages for your own use. Please ask us before using them in any other way.

Links to other sites

When we link to another site, such as a map for directions, we do not control that site and are not responsible for it. A link does not mean we endorse the site.

The site as it is

Details such as the insurance plans we accept, our hours and our services can change. Check with us, and with your insurance plan, before your visit.

We work to keep the site accurate and available, but we cannot promise it will always be error-free or online. As far as the law allows, we are not responsible for loss caused by using the site or by its being unavailable. Nothing in these terms limits the rights the law gives you, or our duties to you as your health care provider.

Massachusetts law

These terms are governed by the laws of the Commonwealth of Massachusetts. Any dispute about these terms or the site goes to the state or federal courts in Massachusetts.

If part of these terms cannot be enforced

If a court decides that part of these terms cannot be enforced, the rest still applies. If we do not enforce a term right away, we can still enforce it later.

Changes to these terms

We may update these terms. The date at the top shows when they last changed, and the version on this page applies from that date. If you keep using the site after a change, you accept the new terms.
